Application - Part 2

Please send the following information:

Questionnaire

Include with the completed application form your answers to the following questions. These answers should be typed (maximum of 5 pages), or recorded on audio or videotape (maximum of 30 minutes).

  1. How will you finance your next year of post-secondary education? If already in a post-secondary program, how have you financed the current year? (Student loans, grants, scholarships, help from parents, part-time/full-time job, savings, etc.)
  2. What jobs have you held? Or what community volunteer work have you performed? And what have you gained from these experiences?
  3. What achievements or accomplishments -- academic, personal, community, or elsewhere -- have made you especially proud? Explain. Examples include volunteer work for HIV/AIDS, cancer or other health care organizations; participation in groups that support transgender rights, participation in local transgender organizations; or participation in allied organizations that otherwise support the transgender communities, such as Parents and Friends of Lesbians and Gays (PFLAG). The strength of the applicant's commitment to bettering the transgender communities is one of the criteria established by the Transgender Scholarship and Education Legacy Fund however, the scholarships will be awarded regardless of age race, sex, national origin, sexual orientation, or other group protected by non-discrimination laws.
  4. How has being identified as transgender affected your life?
  5. What would you like to be doing -- in terms of work, community service, and personal life -- ten years from now? How do you plan to work toward those goals in the next ten years? At this time we are awarding grants only to individuals pursuing a career in the helping and caring professions. Include in the narrative where your interest in the helping and caring professions began and your background in those professions (if any), any significant obstacles you have overcome or expect to encounter (and any special circumstances), and an estimated time line for major career milestones you have established for yourself.
  6. Is there anything else you want to tell us about yourself -- educational and personal goals, activism, and plans?
