Application - Materials Checklist

Applications will be considered complete when the following materials are included:

1. completed applicant information (part 1, sent by you)
2. the questions in part 2 are carefully completed (part 2, sent by you)
3. completed list of your references (part 3, sent by you)
4. two recommendation letters (sent by the people that you listed in part 3)
5. one of the following documents (sent by the school or by your 3rd reference)
   • For currently enrolled high school students: send an official copy of your high school transcript.
   • For GED graduates: send a copy of your GED certificate.
   • For currently enrolled post-secondary students: send an official copy of your most recent transcript.
   • For all others: an official copy of the transcript from the high school, post-secondary, or training programs you most recently attended. If you have not attended any school within the past 10 years, and no transcript is available, please submit a third letter of reference.
     Note that transcripts must be sent directly by the school, or must be an official, sealed school transcript.

The items sent by you (items 1, 2, and 3) must be postmarked by February 1st.  All other items should arrive within 1 week after that those dates.
